Part Time working an average of 17.50 hours per week over a 2 week rota

Salary:       £9,067

Matrix payments for evening and weekend work and car user payments where applicable.

 

 

We are part of the Local Care Organisation (Bury LCO) which was formally launched on 1 April 2019.  Our aim is to enable improvements in population health and wellbeing, by providing improved health and care services to the people of Bury. We are an innovative formal alliance of organisations: Bury Council; Bury GP Federation; BARDOC; Bury Voluntary, Community and Faith Alliance; Northern Care Alliance NHS Group (Salford Royal and Pennine Acute Hospital NHS Trusts); Pennine Care NHS Foundation Trust; Persona Care and Support Ltd.

Our partners have made a commitment to work together as a single new body within the public sector, responsible for the transformation and delivery of a group of services and improving people’s health.

We also work closely with NHS Bury CCG which, with the Council, forms what is known as the One Commissioning Organisation (OCO).

The Bury LCO will work as one joined-up team rather than separate organisations, leading to more coordinated help and support, getting the best value for public money.

We are looking to recruit 7 Wellbeing Practitioners within the Choices for Living Well Service, based at Killelea House, Brandlesholme Road Bury and in the community.

There is an opportunity to work towards and gain Wellbeing Practitioner level 2.  You must be a car driver and have access to a car. Knowledge of working in a similar post would be an advantage.

This is an exciting time to join the service which has recently undergone a refurbishment programme to enhance the existing reablement facility so that people can live more independently for longer. 

 

The Service is a community short term service that supports customers including those with complex needs, to regain their skills and confidence so they can return home after a period of therapy and support.  Working as part of a multi-disciplinary team that can make a real difference to the lives of vulnerable adults living within the Bury area.   You will encourage and motivate customers and assist them to build their confidence and achieve their personal goals.

 

To therapeutically and proactively enable customers to reach their own potential, maintain their independence and continue to live in their own home or return to their own home after a concentrated period of support.

 

You will need to demonstrate enthusiasm, commitment, drive and a strong teamwork ethic and be flexible in supporting colleagues and the service during busy times and periods of absence.  You will be supportive in assisting and promoting the community and bed based service.  The hours of work are over a 2 week rota working an average of 17.5 hours per week.  Rota 1 hours are 12noon to 5pm and Rota 2 hours are 4.30pm to 9.30pm both include weekend working.
