Post:      Wellbeing Practitioner Level 2 working within the Intermediate Tier

Part Time working an average of 35 hours per week over a 2 week rota or

Part-Time 20 hours per week covering two night shifts

Salary:       £10,569 per annum

Matrix payments for evening and weekend work and car user payments where applicable.

 

We are part of The Bury Local Care Organisation (Bury LCO) was formally launched on 1 April 2019, although we have been developing over the last year or so.

The aim of the LCO is to enable improvements in population health and wellbeing, by providing improved health and care services to the people of Bury. We are an innovative formal alliance of organisations: Bury Council; Bury GP Federation; BARDOC; Bury Voluntary, Community and Faith Alliance; Northern Care Alliance NHS Group (Salford Royal and Pennine Acute Hospital NHS Trusts); Pennine Care NHS Foundation Trust; Persona Care and Support Ltd.

The LCO partners have made a commitment to work together as a single new body within the public sector, responsible for the transformation and delivery of a group of services and improving people’s health.

They also work closely with NHS Bury CCG which, with the Council, forms what is known as the One Commissioning Organisation (OCO).

The Bury LCO will work as one joined-up team rather than separate organisations, leading to more coordinated help and support, getting the best value for public money.

Bury is making a significant investment to improve the capacity and capability of Intermediate care and related services under the leadership of the Bury LCO.

 

Working with local commissioners to deliver the integration and transformation of local health and care services is part of Bury’s Locality Plan. Improved Intermediate care will enable more people to have their health and social care needs met at home and in the community and avoid people being treated in hospital or care homes.

 

The new delivery model for the Intermediate tier of services is designed to bring together health and social care into a Bury-wide integrated team delivering services under single leadership. The key aims are:

·       Introduce home-based care into Bury’s intermediate care offer

·       Create additional intermediate care beds at Killelea House

·       Bring the existing intermediate beds from Bealey Community Hospital into the intermediate tier

·      Develop a multi-disciplinary Rapid Community Response Team.

 

  

We are looking to recruit a number of permanent and fixed term contracts until September 2021 of Wellbeing Practitioners within these services. 

 

You must be a car driver and have access to a car. Knowledge of working in a similar post would be an advantage.

This is an exciting time to join the service to enhance existing teams. 

 

The Services are home or bed based short term service that supports customers/patients including those with complex needs, to regain their skills and confidence so they can return home or remain at home after a period of therapy and support.  Working as part of a multi-disciplinary team that can make a real difference to the lives of vulnerable adults living within the Bury area.   You will encourage and motivate customers and assist them to build their confidence and achieve their personal goals.

 

You will need to demonstrate enthusiasm, commitment, drive and a strong teamwork ethic and be flexible in supporting colleagues and the service during busy times and periods of absence.  You will be supportive in assisting and promoting the community and bed based service.  The hours of work for the 35 hour position are over a 2 week rota working an average of 35 hours per week.  These rotas are split shifts 8am-1pm and 4.30pm-9.30pm on a shift pattern, 4 days 1 week and 3 days the following week including weekend working. The 20 hour position is two nights working 10 hour shifts on a Saturday and Sunday.
